Formatting of flashing it seems to be your best options. Because this is clearly NOT normal.
I think that was a problem in old firmwares, try updating to the latest!
if you already did and still have the problem, then there might be a software that is causing this issue, do as Delta737 said formatting id your only choice

guide me guys through this process, i'm planing to do the following:
1. Formatting the phone and the memory card (after backing-up personal information)
2. Updating the firmware.
3. re-installing the Apps that i need and my personal information

Once you update your firmware it automatically formats the phone, so that would be doing more work than necessary!
