SiS file
I have seen the application on a 3650 (it has a little bug as its icon, and is called field test) - it is a lot bigger than the monitor on my old 6110 (took me ages to scroll through all the screens)! I tried to copy the app files to a 7650 but it didnt work, maybe I didnt find all the files. If service centres have access to the net monitor, then I expect they may just have a .sis file which they install to the phone and take off again afterwards.
if you still have access to that phone, could you try finding the .SIS file in one of the 2 following places: either in the "installs" directory in C: or, if it is a "special ROM", in Z: where all other "standard" apps are stored (maybe try to find it with seleq or efileman latest version ... put *.SIS 😊)

Or you could just do a "full backup" with a PC suite (if supported)

a .sis file is a package (like a zip file) and when installed the compponents are unpacked and installed as seperate files. So without the .sis file, it is difficult to know if you have found all the individual files on the phone.
