I have a new QD that I synced for the first time this morning, and it worked fine. I've had trouble with the QD in the past though (this isn't my first unit owned). My original QD kept giving me that same error -- I believe -- and I ended up exchanging it, and the second unit worked fine.
However, my Nokia 3650 used to work flawlessly, and now it gives me the same error you are reporting during a sync. However, the sync still works after I hit the button and move past the error. Did you try that? Does it fail after the error popup closes?
What version of iSync are you using? What verison of iCal? If you have the latest updates for both, I would expect you to be okay. iSync is at 1.5; I forget the verison for iCal. I can tell you that a December update to iCal broke iSync for Symbian phones, but a fix was released at the end of December.
You mentioned reformatting your phone. I know there is the "soft" format *#7370#, which is what I actually did to my 3650 last week. There is also a "hard" format, and I don't know the difference, but you might try that. (Look for details and code on this forum.)
You should also look into doing a repair of permissions on your mac, in case there is a problem with your iSync-related files. I would also try to remove the device from iSync, repair the phone via the bluetooth prefs, then add it again to iSync (i.e. remove it and add it as a new phone). If you do this, then I would suggest checking the option to "erase device and copy from phone."
iSync has a log window viewable from the menu, next time it misbehaves you could see if there is an error.

I used to be able to do it smoothly till I reformatted my phone and MMC card recently. Anyway, every time I try to sync I get a warning on the phone "App. closed AppArcServerThread". What does that mean? After clicking ok, my contacts sync well, but my calendars and To Do items fail to sync...
Any solution? Some software incompatability perhaps..
