mmc formatting guidance

Hi all i cant format my mmc card 128mb it gives me an error "App. closed AppArcServerThread" tried to format it on another n-gage phone the card seems to be locked only works on my phone and dunno whats the password any tips suggestions well be great

Also on mine site u can find tool for restoring forgotten/lost MMC password...
