If you have blocked your phone. You need to phone your service provider for the PUK code.!!!

Right, the PUK (Personal Ublocking Key) is related to the SIM card (and its serial number, IMSI; International Mobile Station Identifier) and only your operator (= carrier = service provider) can give you the code. So, call their customer service - with another phone, obviously. 😉
