First of all, the files which r very important to u will be erased if u somehow manage to format.

If possible for u, get back ur 3650 for some time. Install FExplorer from http://www.gosymbian.com
Go to C:/System.
Select a file named 'mmcstore' and press 'Options>File>Hex/Text Editor'.
Now the password will be visible along with some unclear text.

If that doesnt work or u cant bring ur phone, formatting can be done using a 9210i Communicator. Not sure about it though.
Borrow a 9210i from somewhere. Enter ur card in it and switch it on. A popup screen will ask for password automatically. Ignore it. Then go to file manager. Press Menu on the corner right bottom. Now go to memory card and choose format memory card.

Im not sure, but heard of it. But remember, formatting will remove all the data in ur card.
