how to hard reset?

guys, do you know how to hard reset and the code too?? my comunicator can,t detect my MMC. if can detect, it's so slowly. For example after i input my MMC to my 9500. i go to file manager.there's no my memory card. After more than 1 hour, my 9500 detec it.i don't know why? My latest firmware is V 05.22(01). Help me guys 😃

ctrl +shift+f while device poweron

First Remove the Battery.. then Put it back to make a reset..
..then while Your phone is Booting up Press n hold ctr + shift + F .. then it will ask you if you want to format your phone...
