how to unlock my mmc

hi,i put a pasword on my mmc,later it refuses to take that pasword then i format my multimedia card and NOKIA-3650 phone,but still i am not able to get my mmc unlock so please helpl me in this

Do a full format on PC with a card reader, not on phone.
