Video problems

I have a Nokia N95 and a 2GB Skandisk Micro SD card.
I've watched a few video files on my phone over recent months.
Recently I've noticed something happen which is preventing me from watching video.
I have video files I've converted to mp4 and transferred to the memory card.
Usually the first few minutes are OK, but then after a few minutes the image starts to break up, and when it finally gets back together OK, the audio is no longer in sync with the video- not just a split second out, but totally out.
I thought for a while it might have something to do with the original files and how I'd converted them (without going into detail I used to use 'Super' but since converting from Windows to Linux (Ubuntu) I use a program called avidemux), but I have downloaded mp4 files direct from certain websites which didn't need converting and I've still had the same issue.
So what could be the fault? Is it a memory card issue? A file issue? Something to do with the phone itself?
Any suggestions?
