Out of curiosity, did anyone require a firmware update on their phone in order to use the larger MMCs (I have a 6680 with the stock 64M)?

I've 6680 with 256 Kington DV MMC without any upgrade of firmware, I don't think you need it, nokia phone aren't limited like some sony ericsson (P900,..)

just got mine 512mb. not sure do i have to formatt it before i use it, if so on the phone or pc

Just do it on the phone 😊 Its a good precaution IMO before you get started.
